Scheduling your visit: Both new and established patients can schedule their own appointments by clicking the button below. You will asked to provide a credit card, but it will not be charged or credit-checked.

Insurance: We do not accept insurance or Medicare, but will provide you with appropriate forms for possible reimbursement from your insurance company. We will not fill out “prior authorization forms” for any services or medications. These are the responsibility of your primary care physician or other specialist. We can accept most HSA or FSA cards for full payment.

Cancellation Policy: You should get email or text notifications of upcoming appointments, if not, please contact us. You may cancel up to one business day prior to your appointment. All cancellations with shorter notifications or no shows may incur a $100 charge.
