Insurance: We do not accept insurance or Medicare, but will provide you with appropriate forms for possible reimbursement from your insurance company. We can accept most HSA or FSA cards for full payment.

Cancellation Policy: You should get email or text notifications of upcoming appointments, if not, please contact us. You may cancel up to two business days prior to your appointment. All cancellations with shorter notifications or no shows will incur a $100 charge.
